availability. Please provide 24 hours’ notice if you cannot keep an appointment.
Walk-ins welcome for sick patients established with our practice. Please be at the clinic by 11:00 a.m. to be seen before noon, and arrive no later than 4:00 p.m. to be seen before the end of day.
Payments Accepted
Sunshine Health (Medicaid), Simply Healthy Kids, Tricare, ChampVA, debit or credit cards, and cash are the payment methods currently accepted by our clinic.

After 26 years of serving Williston and surrounding communities, we will be closing our doors permanently on or before October 15 th , 2026. We thank the community for its support over the years and wish everyone good health for the years to come.
Medical records can be obtained from our office until we close our doors. While we have set a date of October 15th to close, it may be necessary to shut down before then. The request for medical records must be made in writing, and a form is available at our office for this purpose.
After November 1st, medical records can be requested from our medical records custodian, Emerald Document Imaging. The requests can be made in writing at:

However, their preferred method of request is to complete a form through their website here. Once the form is filled out, they will contact the authorized representative and notify them of the fee. The fee is typically $25 per chart. They will only release whole charts. Further questions about their services can be answered by calling (631)435-2900. Medical records will be available through Emerald Document Imaging for seven (7) years.
